Springfield Healthy Hearts uses real-world data to understand how heart health is changing across the community over time. By combining community insight, clinical data and system-level trends, we measure progress, identify gaps and guide action where it is needed most.

Our measurement approach

We take a whole-of-system view of heart health.

Our approach brings together multiple data sources to build a comprehensive and evolving picture of health in Springfield. This includes:

  • Community-level data from the Springfield Study (surveys, health checks, biospecimens)

  • Health service data (e.g. hospital presentations, service use)

  • Population-level indicators and emerging data sources

  • Community insights gathered through engagement and co-design

By integrating these inputs, we move beyond isolated metrics to understand how health is shaped across the system.
